Since 1991, the English National Health Service (NHS) has been subject to several control\ud and management regimes that have often used market based approaches to enable the\ud delivery of services. This paper uses survey based research to explore the resistors and\ud challenges that impact on the development of effective relationships between commissioners\ud and providers in delivering high quality patient care.\ud The study is part of a larger multi method study and although this paper focuses on the\ud quantitative aspects it is the authors’ intention to publish other aspects of the study in the\ud future.\ud The findings highlight the key challenges as the short tenure of senior leaders, time limitations\ud on organisational stability, prescriptive performance management frameworks, lack of\ud innovation and conflict and tensions between commissioners and providers.\ud The paper enhances the understanding of the complexity of the commissioner-provider\ud relationship and develops a model to assist the understanding of this complexity.
