Resumo: A hipotonia é a diminuição do tónus muscular, de forma generalizada ou focal, com uma vasta etiologia. Este é o caso de uma lactente de dois meses de idade referenciada à Consulta Externa de Pediatria por hipotonia cervical, sendo este o único achado ao exame objectivo. Apresentava antecedentes de internamento num outro hospital às três semanas de vida por dois supostos episódios convulsivos em apirexia, tendo sido efectuado estudo metabólico e de enzimas musculares, sendo ambos normais. A ressonância magnética (RM) raquidiana que revelou um hematoma epidural desde a vértebra cervical seis (C6) à vértebra torácica dez (T10), tendo sido submetida a laminotomia, com recuperação neurológica completa imediata. Dada a raridade desta patologia, salienta-se a importância da sua suspeição clínica na abordagem diagnóstica da hipotonia.
